Clear Coverage will be retiring this weekend (12/5/2020). We are replacing it with a new tool called ProAuth which we believe you’ll really enjoy. ProAuth is scheduled to launch later this month. In the meantime, if you need a prior authorization for imaging services, please submit one via fax using this form.
